How do I add/change a beneficiary?

Online: To designate or change a beneficiary for all of your accounts (except IRAs; contact our Member Service Team at (800) 874-5544, Monday through Friday, 8:30 a.m. to 7:00 p.m. ET to request an IRA Beneficiary Form):

  1. Log in to online banking.
  2. From the Dashboard, select "Account Services," then "More Services."
  3. Under "General" select "Manage Beneficiaries."

Other Options: Fill out this Designation of Beneficiary Form and submit all required documentation via one of the following channels:

  • Secure Message: Compose a secure message within online banking, with your completed documents attached
  • Fax: (914) 641-3730, Attn: Operations
  • Mail: Quorum FCU, Attn: Operations, 2500 Westchester Avenue, Suite 113, Purchase, NY 10577

Complete all applicable fields in the Designation of Beneficiary form, and note the following:

  • Provide a copy of a government-issued photo ID (e.g., license, passport) that includes the signature of the primary owner of the account.
  • If you do not indicate a suffix on the form, the beneficiary(ies) listed will be added to all current checking and savings accounts on the membership. (Please note that a new beneficiary form will be required for each savings/checking account opened once this form is processed.) The suffix is the four-digit number that appears before each account name on your monthly statement, and after the dash under each account name in online/mobile banking.
  • Total combined share % (percentage) for all beneficiary(ies) cannot exceed 100%.
  • Social Security Number, Date of Birth and Address fields must be completed for each beneficiary.
  • The form cannot be signed electronically (e.g., e-signature or PDF signature); a physical signature is required.
