How do I change the name on my account?

To change your name on your account, simply complete our Membership Change Form. Please complete the form, taking care to complete all applicable fields, and note the following:

1. Provide a clear copy of valid government-issued photo ID (e.g., license, passport)—which includes a signature. If you are removing a Joint Owner, you must provide a clear copy of a government-issued photo ID for the joint owner you are requesting to remove.
2. There are three locations on the form where a signature is required.
3. The form cannot be signed electronically (e.g., e-signature or PDF signature); a physical signature is required.
4. If the form is being completed for a name change, please provide a copy of supporting documentation (i.e., marriage license, divorce decree or certificate of name change).

Please return all documents, including a copy of photo IDs, to us via one of the following ways:
